9:10 AM Presentation: Coordinating Care: Designing, Implementing, and Measuring The Impacts of Different Rounding Modules Across 8 Hospitals

This presentation describes Houston Methodist's system-wide initiative to improve care coordination and patient experience across eight hospitals through the design, implementation, and evaluation of standardized rounding practices. In response to historically lower performance on care coordination measures and significant changes to CMS HCAHPS scoring in 2025, the organization developed and deployed structured multidisciplinary rounding (MDR), physician-nurse (MD/RN) rounding, and bedside shift report models aimed at strengthening communication, teamwork, and patient engagement.

The session highlights best practices for effective rounding, including clearly defined team roles, patient and family involvement, visible interdisciplinary collaboration, and consistent communication of the plan of care. Participants examine common barriers and ineffective rounding behaviors through case studies and discussion. Early outcomes demonstrate improvements in patient perceptions of communication, coordination, and teamwork, with positive feedback indicating greater understanding of care plans and increased confidence in the healthcare team. The presentation offers practical strategies and lessons learned for healthcare leaders seeking to operationalize rounding programs that enhance care coordination, patient experience, and overall quality of care.
